This role involves assessing the health needs of individuals, families, and communities, and providing comprehensive nursing care focusing on preventative health, health education, and chronic disease management, primarily within a community setting.
Duties and Responsibilities include but are not limited to:
- Assess the health needs of individuals, families, and communities.
- Develop and implement community health education programs and initiatives.
- Provide health education on topics such as nutrition, immunizations, and disease prevention.
- Organize and participate in community health fairs, workshops, and outreach activities.
- Conduct health assessments and screenings, including blood pressure, glucose, and cholesterol levels.
- Administer immunizations and medications as prescribed.
- Provide wound care, first aid, and other nursing interventions.
- Manage chronic diseases such as diabetes, hypertension, and asthma through patient education and follow-up care.
- Develop individualized care plans for patients with complex health needs.
- Coordinate care with other healthcare providers, social services, and community resources.
- Monitor patient progress and adjust care plans as needed.
- Educate patients and families on managing health conditions at home.
- Maintain accurate and up-to-date patient records.
- Document patient assessments, care plans, interventions, and outcomes.
- Ensure confidentiality and compliance with health information regulations.
- Participate in hospital emergency preparedness planning and response activities.
- Participate in continuing education and professional development activities.
- Any other duty as maybe assigned by the supervisor from time to time
Qualifications
- Diploma/Bachelor of Science in Nursing (BSN) or equivalent from an accredited institution.
- Minimum of 2 years of nursing experience, preferably in community health, public health, or a related field.
- Experience in health education and promotion activities.
- Current Registered Nurse (RN) license with NCK.
- Basic Life Support (BLS) certification will be an added advantage.
