Job Description
As a Medical Social Worker, you will be responsible for providing psychosocial assessments, counseling, and crisis intervention services to patients and their families. This role involves patient advocacy, care coordination, discharge planning, and directing support initiatives. Additionally, you will coordinate and manage activities for the Endometriosis Foundation, promoting educational resources and fundraising.
Key Responsibilities
Patient Care & Psychosocial Support
Conduct comprehensive psychosocial assessments to identify patients' emotional, social, psychological, and financial needs.
Provide counseling and emotional support to patients and their families throughout treatment and recovery.
Deliver crisis intervention services during medical, psychological, or personal emergencies.
Facilitate difficult conversations, including end-of-life care planning, in collaboration with the multidisciplinary healthcare team.
Organize and facilitate patient and caregiver support groups to promote emotional wellbeing and reduce social isolation.
Patient Advocacy & Care Coordination
Advocate for patients' rights and ensure equitable access to healthcare and support services.
Coordinate discharge planning by working with community organizations, rehabilitation facilities, and home healthcare providers.
Connect patients and families with financial assistance programs, housing support, mental health services, and other community resources.
Assist patients in accessing specialized healthcare services and navigating healthcare systems.
Endometriosis Foundation Coordination
Coordinate and manage activities of the Endometriosis Foundation.
Mobilize financial resources and support fundraising initiatives for the foundation.
Develop educational materials and resource directories on endometriosis treatment, pain management, and patient rights.
Educate patients on available healthcare services, workplace and educational accommodations, and available support systems.
Represent the foundation during awareness campaigns, community outreach programs, and public engagement events.
Documentation & Compliance
Maintain accurate, confidential, and up-to-date patient records in accordance with hospital policies and healthcare regulations.
Document assessments, interventions, counseling sessions, referrals, and patient outcomes.
Participate in multidisciplinary team meetings, clinical case reviews, and patient care planning sessions.
Qualifications & Experience
Education
Bachelor's Degree in Social Work (BSW) from a recognized institution.
A Master's Degree in Social Work (MSW) will be an added advantage.
Experience
Relevant experience as a Medical Social Worker within a hospital or healthcare environment is preferred.
Experience in acute care, rehabilitation, maternity, women's health, or long-term care settings will be an added advantage.
Experience coordinating community outreach or health-related foundation programs is desirable.
Knowledge, Skills & Competencies
Strong understanding of medical terminology and healthcare delivery systems.
Knowledge of community support services and patient referral networks.
Excellent counseling, communication, and interpersonal skills.
Strong problem-solving and crisis intervention abilities.
Ability to assess patients' psychosocial needs and develop appropriate intervention plans.
Proficiency in Electronic Medical Records (EMR) systems and clinical documentation.
